Required Medical Form for participation in any sports programs at Saint Charles.

This physical form must be signed by a doctor and submitted to the St. Charles school office to be eligible to participate in any sports programs during the current school year. One form per school year will cover participation in all sports. The form must be submitted prior to the start of the sport your child will be playing. Your child’s doctor will complete the form if they had their annual physical in the past year. Therefore, you should not need to make a separate appointment to get this form completed if your child has been seen in the past year for their annual physical.
