Fill in all the correct details with the attached requested documents.
Bring or send your completed information (including a copy of your child’s birth certificate, immunization records, and baptismal certificate, if applicable)

The 2024-2025 tuition assistance application window closes May 16.
Tuition can be reduced by participating in the St. Charles Gift Certificate Programs
